If I didn't want to rock the boat, I'd get something on the list.
However, sometimes it is really fun to play passive/aggressive with nutty IL's, so I'd probably get something really odd like say a crescent wrench, a can of whipped topping, and a box of envelopes.

Why would she want to return something that is on the list? Oh wow. I will not complain about my mil who asks for lists, and then doesn't usually buy anything off the list. My only problem with that is, I give her a list of ideas I could have used mysef, and then the kids don't get what they asked for. One year she gave my 14 year old the gift that the 13 year old asked for.
I know how you feel about gift cards, but someone who returns what she asked for deserves a gift card.

Last year I got each of the kids a board game from WalMart. I got them on sale for $5 each. I included the gift receipt so she could return them, of course.
She called me from WalMart and said "I'm here at WalMart returning these games and they're only going to give me $10!". So I said, well that's what I paid for them.
She got really irritated because "what in the world was she going to buy at WalMart for $10"! I didn't say anything but I could think of a few things...
I think she is a very unhappy person. It could be that she never wears a bra. Maybe her boobs make her sad? Lift the girls up! Be Happy! Darn it, now I feel kinda bad for her! LOL!
She really just isn't nice. My BIL (her Dh) has celiac disease. My MIL passed it on to two of her 3 sons. My Dh didn't get it, and neither did my kids. She has said some mean things to him, like when we found out that he had to go on a cholestrol lowering drug since all the exercise/diet wasn't working and our doc felt it was hereditary. She said to Dh "well it's about time something bad happened to you".

Gad she is like my sister.... We draw names and she emailed me because I got her and she got me that we sould just keep our $50.00 I wanted to cry. Now she won't even give me ideas ( we usually do a little wish list) So I am trying to get creative now on what to get her to just piss her off. That $50 will be worth it.
As for your situation. I would just keep getting her what ever I wanted to get and not let it bother me. I would include a gift recipt and if she kept saying what she says out load I would prob try to catch her when no one else is in the room and have a little kids learn from example type thing and ask her not to say thoose things infrom of my kids becuase I don't teach them to be ungrateful.

My MIL has the opposite problem!  She will give the kids everything I put on the list. I always tell her it is just a list of suggestions but they get it all.  I gave tried shorter lists but she just adds on some things they really don't need so I give her a long one. It sure saves us money!

She returns things that aren't on her list...so if you buy anything else, she returns it. I like the list, but sometimes I just find something I think she'd like or her kids would enjoy. Nada. She returns it.
