The first molars are dreadful. There's a lot of surface to break through. The subsequent ones really aren't bad. It's like they get used to it. It really does sound like you're doing everything, but another thing to think of is it's possible her ears are hurting, and she could have an ear infection, what w/the fever. May be worth letting the Dr. look at them - I didn't even know the last few times dd had them, because she was not feeling well anyway, and there are nasty colds going around.
My son was extra sensitive during teething - we had to have dentist strength Anbesol (20%!), but you would have known that by now w/the earlier teeth.
Hylands tablets help w/teething, but aren't long lasting. Also, if it's the ears, the pharmacy has some OTC drops that numb the pain. Some swear by garlic oil. If the doc says it is an infection, ask for an rx for the rx drops - some don't offer them until you ask.
A frozen wash cloth or frozen peas helped with my son's molars, too. Good luck!
