Being that I work for a Physicians office & get this same complaint ALL THE TIME, I would like to offer some explanation for the fee.....which is the same information I tell all the patients.....the fee charged by your doctor for the completion of any insurance forms, disability forms, FMLA forms, etc. are not decided by your physician, but by the state you live in!!! The fee varies from state to state, but EVERY state carries the same law that physicians are to charge that fee for the completion of those forms. The reason being is that most often there are more than 1 form to complete & almost every form is more than 1 page. The forms are lengthy & complex in that you must research through the patients medical history (chart) to provide correct explanations & documentation, with most requiring you to include copies of medical records The fee covers the cost of the employee's time to complete the form and the cost of paper, ink, machine, etc. supplies to provide copies of records. And, also because any insurance/disability, etc. forms are not relevant to the care & treatment that your doctor is providing to you, but for the contract that is held between you & your insurance company! In fact, physicians are not REQUIRED to complete the forms & many physicians offices REFUSE to do so. Just think, an average 2-Doctor Surgical Practice gets at least 50 of those forms each week...thats a lot of time to fill out alot of forms!!!
So, I tell my patients that ANY & ALL complaints should be directed to the STATE LAW-MAKERS, as we are just complying with the law ourselves.

Oh, and I was shocked to hear that the fee was only $20....I live in Alabama & work in Georgia (we are just across the river) & both state's fee's are $35!
