For this one, it "requires" a business name. But I just put "N/A" in the blank and it went through.

Also, towards the bottom, it asks you if this is a P.O. Box and/or Residence. You have to select "yes" or "no" on each question.

Oh, and it would not accept "N/A" in the blanks where it asks "How many recipes will we collect" and "How many cook books will be printed". I put "50 in each space.
