After you take survey for birth control, you can opt for the $10 gift card and leave email and then they contact you in 4 -6 weeks I thought we could do it right away. Oh well, thanks for the post . I will see if I get the email.
fyi if you read all the info on this survey the survey part is not all to this catch lol hope you guys gave the correct phone # cause they suppose to call you in a few weeks to do another survey over the phone just thought you may want to know

Actually they don't ask for a phone number, just an email address. So I assume the follow up survey will be online. I hope it doesn't go to my spam folder.
