How do you make monogram cards?

Are we going to do these for the first name or last name? I have seen them both. I am up for either or!! I will keep track of the letters and post them all on the Swap #14 thread I am going to start.

This is a sample of a monogram card that a friend of mine made for me. This is not the EXACT card she made me as you will notice it says "Baby". :D This is a copy of the one that she swapped at the Stampin' Up! Convention in July.

The reason I asked for a list is because some people prefer the monogram for their first name and others for the last name.
