The differences between a 4 and 4T, or a 5 and 5T I beleive are that a 4 is longer than a 4t, but a 4t has more room for a diaper. That is what I was told anyway. I know sizes are probably different for boys, but she went from a 4T to a 5 it seems, kinda skipped the 4s and 5ts. It all depended on the brand. There have been 5s in pants that have seemed a little big, but she has a 5 dress that was really short. You might want to take him to a store to try some things on just to see. Fall clothes should be out now. He may not like it, but maybe you could bribe him with ice cream afterwards. I am thinking you are going to probably have to get different sizes in the top and bottom.
And if you decide to get any second hand clothes, they usually run smaller because they have been washed several times.

Shane went from a 5T to a five. He is a skinny tall twig. He was in a 5 for about three months and went to a six slim. You are probably going to have him try them on just to be sure if you go to different stores.

My advice ... wait. Kids tend to stay in size 5 for a long time. Then he'll have a growth spurt and become taller so the question about which size will become self-evident. My DD went from 5T directly to a 6 slim.

My son, nieces & nephews all went from a 5/5T into a 6. My nieces, being the teeny weeny girls that they are, went into 6 Slims. My nephews, being the "chunky-monkeys" that they are, went into 6X b/c the 6X are a little wider in the waist than a regular 6.
