I tore with both of mine. My first was a fourth degree tear, because forceps had to be used - pretty darn big, and my second wasn't as bad. My doctors both times preferred to let me tear, as they said that the jagged edges of a tear would heal more quickly and better than being cut in an episiotomy.
Regarding your question: I would talk frankly with your doctor. They actually had to stitch me up so much the first time that things were a little too tight, if you know what I mean, and I had to exercise/stretch it a bit. But they did tell me that if it didn't work, I could have minor in-office (w/local anesthetic) surgery to correct it. You may be swollen for up to a year down there from the trauma, so be aware they may want to wait a bit.
Either way, I would bring it up to the doctor ASAP and have them check you out. I had to be checked out a few times, and even sent to physical therapy - yes, I had to work it with a physical therapist. But mine was an extreme case. Your Dr. can help you.
