Car seat - I like the travel system (car seat/stroller)
Highchair or reclining booster seat - I used a highchair with #1, but the reclining booster seat with #2 & #3. It's about half the price and works just fine (as long as you have the extra chair at the table)
Bedding - either a set or go cheaper & just get a bedskirt, sheets & bumper pad
Spit rags - I like to use the cloth diapers, they're thicker than most "spit rags"
Diaper bag - you may get one free from the hospital, but you may want a nicer one - think long and hard though - do you want a regular bag, messanger bag, backpack, or just a really big purse ?? I'm getting by with a smaller bag now since I'm nursing, but once the baby needs food I'll have to move up to my bigger one.
Sling/Baby carrier - there are directions online to make your own sling if you have a sewing machine. (You can also make your own nursing cover-up if you plan on brest feeding - although a blanket works fine too)
And of course the basics...
Diapers
Wipes
Pacifiers
Bibs
Bottles & formula - if formula feeding
Receiving Blankets - I used a TON of these
Breast Pump - if nursing (although not a necessity - I've never used one)
I could go on and on...
