Once every two weeks I just throw it in the washer along with my towels.

If you wash it often, than mold doesn't get underneath it. You can also just hang it to dry over the side of the tub and this keeps mold from growing too. But I like to wash it with the towels.

I don't use one now because our tubs are made with the slip-ressistant stuff in the bottom. But growing up we used a mat. After each bath, we had to pull it up and hang it over the side to dry. Every week it got washed as well. We never had a problem with mold on it unless we forget to pull it up and let it dry after the last shower/bath of the day.
